Reticulocyte Production Index (RPI) Calculator

The reticulocyte production index (RPI) shows whether the bone marrow is making enough new red cells for the degree of anemia. It corrects the reticulocyte percentage for the hematocrit and for the longer time young red cells spend in the blood when anemia is severe. This tool uses the formula and maturation times printed in an open-access study. It is written for clinicians and is not a diagnosis.

Source

The formula and the maturation times as printed by Belo L and colleagues, Renal Failure 2019 (Methods), and the reading of under 2 and over 3 from the review by Nairz M and colleagues, Wiener Medizinische Wochenschrift 2016 (Fig. 2). Both are open access (CC BY 4.0). The tests check the formula, each maturation band, the hematocrit values the bands leave out, the cut-offs at exactly 2 and 3, and that a value near a cut-off is never shown as the cut-off.

Validated in

Adults. Belo 2019 used the RPI in 246 adults on long-term dialysis and 32 healthy controls in Portugal; it is a measurement formula, not a score validated for an outcome. Nairz 2016 is a review of iron deficiency and anemia of inflammation.

Limits

The RPI was developed in adults; a study in babies notes it may not suit children, whose normal blood values and reticulocyte maturation times differ (Ringoringo 2023). The printed maturation bands leave some hematocrit values without a maturation time (above 45%, and 15 to 16%, 25 to 26% and 35 to 36%); for these the tool gives no RPI. The source gives no reading for values from 2 to 3.

The formula

RPI = (reticulocyte % / maturation time) x (hematocrit / 0.45) (Belo 2019). With the hematocrit entered as a percentage, the second part is hematocrit / 45.

Maturation time, the days young red cells released from the marrow circulate as reticulocytes (Belo 2019):

Worked example: reticulocytes 6% and hematocrit 30% give 6 x (30 / 45) = 4.0, divided by a maturation time of 1.5 = an RPI of 2.67.

Reading the result

Nairz and colleagues place the RPI in a stepwise approach to anemia: the reticulocyte count separates hyporegenerative anemias (disorders of red cell production and maturation) from regenerative anemias (hemolysis or bleeding), and the RPI should be calculated to account for the higher share of reticulocytes in anemia and for reticulocytes released early into the blood (Nairz 2016). Iron deficiency anemia is typically a hyporegenerative, microcytic, hypochromic anemia (Nairz 2016).

The tool bands the unrounded value. Exactly 2 is not under 2 and exactly 3 is not over 3, so both are shown as 2 to 3.
